Remove stale host from known_hosts

Useful when a server is rebuilt and you get host key verification errors.

Removes a specific server entry from ~/.ssh/known_hosts. Needed when a server is rebuilt (new SSH host key) or when you get a "REMOTE HOST IDENTIFICATION HAS CHANGED" warning.

When you need this: After rebuilding a server or when SSH warns about a changed host key.
